EPHO: Output phase loss

Cause:
EPHO: Output phase loss typically occurs when one of the three output leads (U/T1, V/T2, W/T3) has become disconnected from either the drive or the motor.
Clearing:
Verify that all three output phases of the ASD are connected at both the drive and the motor terminals. Inspect the motor and driven equipment for issues prior to clearing any faults. Use one of the following methods to clear the fault.
• The [STOP/RESET] button on the AS3 keypad was pressed twice in quick succession
• Activate a digital input that has been programmed to “Reset” function
• Set the tyP (F007) Parameter to: 4: Clear past trips
